Health Insurance

We accept most health plans. We recommend that you call your health insurance provider to verify coverage at our office and with our physician before your visit.

Billing

As part of our patient services, we submit claims to most major insurance carriers. Because of this service, we need you to bring your insurance card and driver's license to every visit to ensure our records are accurate and to prevent any unexpected charges. Please also notify us right away with any changes in name, address, telephone number or insurance provider.

For your convenience, our office accepts cash, check, debit and all major credit cards for your out-of-pocket payments. Our practice is to collect your co-payment, deductible, and any balance on your account at the time of check-in. Patients without insurance pay in full at check-out, unless prior arrangements have been made with the office manager.

Referrals

Please allow 48 hours for referral processing. If you are being referred, please be sure to bring the referral with you at the time of office check-in.
